Almost 300,000 individuals have committed to donating their organs, marking a significant achievement in public involvement, according to the Union Health Minister.

In 2024-2025, India made a significant leap in the realm of organ donation and transplantation, achieving a record of about 18,900 organ transplants, ranking third globally behind the US and China [1][2][3]. Over 3.3 lakh citizens pledged to donate organs in 2023 via the National Organ and Tissue Transplant Organisation (NOTTO), reflecting successful awareness and sensitization campaigns.

Despite this progress, India's deceased donor rate remains low, with an organ donation rate still under 1%, leaving a significant gap between the number of patients waiting for organ transplants and the number of available donors [1][2]. As many as 63,000 patients need kidney transplants and 22,000 require liver transplants [1].

To support patients financially, the Indian government provides assistance up to ₹15 lakh under the Rashtriya Arogya Nidhi for poor patients and covers kidney transplantation costs under the Ayushman Bharat Pradhan Mantri- Jan Arogya Yojana (AB PM-JAY) scheme [2]. Additionally, post-transplant patients receive ₹10,000 per month to help with medical expenses [2].

Efforts to increase awareness and donor numbers include the Aadhaar-based NOTTO online pledge platform launched in 2023 and campaigns such as “Angdaan-Jeevan Sanjeevani Abhiyan” promoting organ donation as a noble humanitarian act. States like Tamil Nadu and Telangana have been recognized for high donor rates and institutional contributions [2][3][4].

Infrastructure development is underway, for example, Karnataka plans to establish organ transplant and retrieval centers in 23 medical colleges to improve organ retrieval and transplantation capacity [2][3][4].
